Psalm 100 ESV
A Psalm for giving thanks. 1 Make a joyful noise to the Lord, all the earth! 2 Serve the Lord with gladness! Come into his presence with singing! 3 Know that the Lord, he is God! It is he who made us, and we are his; we are his people, and the sheep of his pasture. 4 Enter his gates with thanksgiving, and his courts with praise! Give thanks to him; bless his name! 5 For the Lord is good; his steadfast love endures forever, and his faithfulness to all generations.

10:15 Lord’s Supper Form

Beloved in our Lord Jesus Christ,
In this service our Lord calls us to His table to have communion with Him as well as with each other.
Let us listen to what the gospel tells us about the first celebration of the Lord's Supper:
In the night before He went to the cross,
"Jesus took bread, gave thanks and broke it, and gave it to His disciples, saying, `Take and eat, this  is My body.'
Then He took the cup, gave thanks and offered it to them, saying,
`Drink from it, all of you. This is My  blood of the covenant,
which is poured out for many for the forgiveness of sins.
I tell you, I will not drink of this fruit of the vine from now on until that day when I drink it anew with you in My Father's kingdom'."
It is the will of the Lord that all believers join in this communion.
For although it is true that our sins
Make us unfit to come into His presence,
it is just as true that Christ offered Himself as a payment for all our sins.
Surely, this is what the Supper teaches us.
All who trust in Jesus Christ, the Crucified and Risen, are accepted by God in grace.
They may share at this table the communion with the Lord Jesus Christ and with all who, in trial and persecution, look with uplifted head towards the great Day of the Lord.
However, all who trust in themselves for their peace with God,
and those who do not repent of their sins,
those who do not desire humbly to live in obedience to God and in peace with their neighbour,
are urgently called to seek the newness of life which God's grace can give.
Otherwise they should not come to this table lest they eat and drink judgment to themselves.
The bread and wine represent the body and blood of the Saviour.
They are received in faith as signs and seals of all the benefits of His sacrifice on the cross.
The bread and wine are an illustration and guarantee to believers of the forgiveness of sins
of the strengthening of our faith,
and of the communion of believers with the Lord as well as with each other.
These signs and seals of the covenant of grace declare that God is faithful in fulfilling the promises of that covenant, they call upon us to live as God's children in this world and they encourage us to look forward with joy to the Day when God will be all in all.

In breaking and distributing the bread, the minister shall say:

The bread which we break is the sharing of the body of Christ.
Take it, eat, remember and believe that the body of our Lord Jesus Christ
was given for a complete forgiveness of all our sins.

In serving the cup, the minister shall say:

The cup of thanksgiving for which we give thanks, is the sharing of the blood of Christ. Take it, drink from it, all of you, remember and believe that the precious blood of our Lord Jesus Christ was poured out for a complete forgiveness of all our sins.
